Patient-Centered Medical Clinics

(PCMH)

Clara Barton Medical Clinics Are Now PCMH Certified

All three Clara Barton Medical Clinics in Hoisington, Great Bend, and Russell recently received certification as Patient-Centered Medical Homes by the National Committee for Quality Assurance (NCQA).

The Clara Barton Clinics are the first hospital-based Rural Health Clinics in the Central Kansas Region to have received this designation. This new certification establishes a model of care that is patient-focused, improving the quality of care, enhancing patient experience, building better relationships between patients and providers, and providing additional clinic services to better serve patient needs.

How it Works

Patient Centered Medical Home (PCMH) is a care delivery model that uses evidence-based, patient-centered processes, focusing on highly coordinated care and long-term relationship building.  This advanced model of primary care uniquely addresses all of the patient’s medical needs in one convenient location. The primary care provider helps to coordinate all specialty appointments including dental, eye, and mental health together with primary care to ensure the patient’s entire health is at its best. Research shows this model also improves quality, the patient experience and staff satisfaction, while reducing health care costs.

What to Expect During Exam

The process of a Hologic Genius™ 3D Mammography™ exam is the same as your conventional 2D exam. The technologist will position you, compress your breast, and
take images from different angles. For an exam proven to be more accurate, there’s no additional compression required with the Genius™ exam. The technologist will view the images of your breasts at the computer workstation to ensure quality images have been captured for review. A radiologist will then examine the images and report results to either your physician or directly to you.

From the patient’s point of view, very little is different during the actual procedure of a 3D mammogram compared to a 2D mammogram. Most woman will have the opportunity to use the curved paddles designed for patient comfort and experience a similar scan time, but the main difference lies in the superior imaging.
